Full-Time

• 6PM-6AM (Weekend Package - Additional 16 hours added per pay period )

Responsibilities

• Assure that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process

• Develop and communicate individualized plan of resident-centered care in collaboration with the resident/responsible party, family, and care team

• Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes by utilizing all appropriate and available resources for resident/family/responsible party counseling and discharge planning

• Work in collaboration with physician and/or other health care professionals by sharing information relevant to changing plan of care

• Supervise and lead CNAs to perform safe, compassionate, and high-quality care

• Perform accurate and efficient charting

Requirements

• Applicants must be a Registered Nurse (RN) in good standing with the state and maintain a current certification throughout employment
