Responsibilities

Essential Functions

Performs phone management with patients' parents, chart review, care coordination, triage, and problem management. Discusses patient and family concerns over the phone and/or via the electronic health record. Provides and develops patient/family education materials surrounding common Gastroenterology disorders.

Communicates adjustments in treatment plan to families. Formulates care plans for specific patients and documents the nursing process in patient's record. Evaluates patient response to interventions and modifies plan of care as needed to achieve health care goals.

Communicates adjustments or changes in treatment plans to patient/family. Develops and participates in patient and family education about care programs. Communicates lab & other test results to families.

Provides medication refills. Assists providers and staff with patient-related paperwork, including letters of medical necessity, DMV forms, school care plans, and pre-authorizations. Maintains a safe and therapeutic patient care environment.

Works closely with triage team and gastroenterology staff to provide high-quality, efficient care to patients and their families. Keeps current on advances in nursing care by participating in educational programs.

• Utilizes knowledge of medications, procedures, infection control, and safety precautions, and initiates appropriate interventions in crisis situations.

Participates in total quality management to positively affect patient and system outcomes to improve care. Partners closely with the EoE (Eosinophilic Esophagitis) Program Coordinator to provide clinical guidance from a nurse perspective, support onboarding, streamline workflows, and serve as an escalation point for complex EoE care. Actively engages with the EoE Quality Improvement Team, including participation in two monthly QI meetings to lead improvement projects, assist in resolving bottlenecks in the EoE clinic, and monitor clinical outcomes.

CommentsThe above responsibilities will be shared with other nurses and medical assistants in the Department. Incumbent will be expected to provide high-quality patient care in an organization focused on patient safety, patient and parent satisfaction, and positive patient outcomes; advocates for patients and families to ensure unique needs are met; collaborates with highly skilled team members dedicated to achieving patient goals and providing world-class care; actively participates in educational opportunities. Education in patient management is ongoing, with support from division practitioners and support staff.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.

Required

Minimum Qualifications

Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.

Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.

Preferred

Some positions may require additional certifications such as Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), or Neonatal Resuscitation Program (NRP) within one year of hire.

This position is patient-sensitive and must fulfill all associated requirements.
