What You’ll Do

As a Hospice Licensed Practical Nurse (LPN), you will

Responsible for frequent and timely communication to the appropriate registered nurse, supervisor and/or other staff members regarding the patient’s condition. Communication includes participating in case conferences and other more immediate methods of communication when indicated. Provides accurate and timely documentation of patient services to reflect the plan of care.

Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self plan of care. Administers medications and treatments as prescribed by the physician and in accordance with nursing standards of practice. Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.

Ensures that equipment, supplies, drugs, biologicals and other necessary items and services are available. Provides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatment.

What You’ll Need

  • Graduate of an accredited Licensed Practical Nurse (LPN) program
  • Active license to practice as LPN/LVN in the state in which hospice care is provided
  • Minimum one year of nursing experience
  • Hospice experience preferred
