About this role
The Family Medicine Nurse Practitioner provides advanced nursing care to patients of all ages, from infants to older adults. This role involves assessing, diagnosing, treating, and managing acute and chronic illnesses, promoting health and wellness, and working in collaboration with physicians and other healthcare providers. Conduct comprehensive physical exams and health assessments.
Obtain and interpret patient histories and diagnostic tests. Diagnose and treat common acute and chronic medical conditions. Prescribe medications and therapeutic treatments within the scope of practice.
Develop and implement individualized care plans. Provide health education and counseling on disease prevention and healthy lifestyle choices. Manage follow-up care and coordinate referrals to specialists when needed.
Perform minor procedures as appropriate (e.g., wound care, suturing). Document patient care accurately in the electronic health record system. Participate in quality improvement and practice initiatives.
Collaborate effectively with physicians, nurses, and other healthcare team members. Maintain current knowledge of evidence-based practice guidelines. Master’s or Doctorate degree from an accredited Nurse Practitioner program.
If APRN was licensed prior to August 1, 1995, master's degree in nursing is preferred. Privileging documents and related credentialing requirements will supersede this job description in defining minimum qualifications, criteria, and scope of practice, if variation between documents exists.

- Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice
- Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice
- State Controlled Substance License (if required by State)
- National board certification as a Family (FNP-C) or (FNP-BC)
