Hospice works best when care feels local.
You'll become a consistent clinical resource for patients and families throughout the Mankato region, helping them navigate symptoms, medications, changes in condition, and important decisions.
You'll also develop relationships within the communities you serve.
Over time, families will know your name. Facility nurses will know they can call you. Providers will recognize you as a trusted clinical partner.
You're not simply completing visits.
You're helping establish *what Superior Hospice care looks like throughout Southern Minnesota*.
*What You'll Do*
- Serve as the primary RN Case Manager for an assigned hospice caseload.
- Complete comprehensive assessments and develop individualized plans of care.
- Manage *pain, symptoms, medications, wounds, and changes in condition*.
- Coordinate care with physicians, social workers, chaplains, hospice aides, therapists, and the interdisciplinary team.
- Educate patients and families about disease progression, medications, comfort measures, and what to expect.
- Build strong working relationships with assisted living, memory care, skilled nursing, and other community partners.
- Coordinate transitions from hospitals and other healthcare settings into hospice care.
- Advocate for patient *comfort, dignity, and quality of life*.
- Maintain accurate and timely clinical documentation.

We're especially interested in nurses with backgrounds in
- Hospice.
- Palliative Care.
- Oncology.
- Home Health.
- ICU or Critical Care.

- Skilled Nursing.

Hospice experience is preferred, but we're equally interested in nurses who bring *strong clinical judgment, independence, communication skills, and a genuine ability to connect with patients and families*.

- Active Minnesota *Registered Nurse (RN) license*.
- Strong clinical assessment and decision-making skills.
- Ability to independently manage a patient caseload.
- Excellent patient, family, and provider communication.
- Confidence working in a community-based environment.
- Strong organization and care coordination skills.
- Valid driver's license and reliable transportation.
- Willingness to travel throughout *Mankato and surrounding Southern Minnesota communities*.
