Responsibilities

Monitor chronic health conditions and reportable changes, ensuring resident safety and well-being

Assist with the admission process, providing resident orientation and discharge planning as needed

Participate in comprehensive assessments, individualized care planning, and development of resident-centered care plans

Provide nursing care that is compassionate, sensitive to individual experiences, and focused on promoting resident autonomy and dignity

Monitor resident weight, hydration, and nutrition, notifying practitioners of changes

Maintain accurate documentation of all nursing care and services provided to residents

Collaborate with hospice services to provide end-of-life care and support

Promote falls prevention practices, provide first aid, and adhere to infection prevention and control practices

Administer medications, report adverse consequences, and provide oversight of Certified Nursing Assistants (CNAs)
