Patient Assessment & Care: Conducts comprehensive assessments, provides holistic nursing care, and notifies physicians of significant changes. Care Planning & Coordination: Develops, directs, and evaluates individualized care plans, including patient/family education and discharge planning, collaborating with interdisciplinary teams. Treatment Implementation: Prepares patients for procedures, implements physician orders, and ensures patient safety protocols, including medication administration.
Ongoing Evaluation & Adjustment: Regularly reassesses patient responses to interventions, adapts care to changing needs, and communicates with physicians for modifications. Effective Communication & Handoffs: Utilizes Bedside Shift Report, provides thorough patient updates, and delegates tasks for seamless care transitions. Safety & Quality Assurance: Maintains a safe patient and work environment, promptly addresses hazards, and actively contributes to quality and patient satisfaction initiatives.
