*Job Summary*
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. *Duties*
* Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
* Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
* Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
* Manage airway care, ventilator support, and acute pain management for patients with complex needs.
* Document all patient interactions thoroughly using Electronic Health Record (EHR) systems while maintaining strict compliance with HIPAA and clinical confidentiality policies.
* Provide wound care, including suturing and dressing changes, ensuring aseptic technique is maintained at all times.
* Support discharge planning and client education to promote ongoing health management in various settings such as long-term care facilities or home & community care environments.
*Skills*
* Proven experience working with diverse patient populations across settings such as long-term care, assisted living, hospitals (including ICU and trauma centers), hospice, or skilled nursing facilities.
* Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10) for accurate documentation and billing purposes.
* Skilled in IV infusion therapy, catheterization, tube feeding management, and airway management techniques.
* Proficiency with EMR/EHR systems such as Cerner or Epic for efficient documentation and patient record management.
* Ability to perform triage assessments quickly and effectively in urgent or emergency situations; experience in emergency medicine or urgent care is highly valued.
* Familiarity with infection control practices and infection prevention protocols to ensure patient safety.
* Excellent caregiver communication skills to collaborate effectively with patients, families, physicians, and interdisciplinary teams.
