We are seeking a Registered Nurse to join our team of home health professionals. The successful candidate will be responsible for providing quality care to patients in their homes, assessing and monitoring patient conditions, and providing education and support to patients and their families. The ideal candidate for this position will have excellent communication skills, an ability to work independently, and a commitment to providing the highest level of care.

Essential Functions

*

As a Registered Nurse you will be responsible for electronic charting, visit multiple patients' homes each day and provide a full range of personalized, acute care, which may include instruction on chronic disease management, medication, and pain management, wound care, IV infusion, and post-operative instruction. You will work with patients to provide the tools they need to ensure their own recovery. Seeing the end results of their recovery is just one of the many rewards of the position!

Responsibilities

  • Provide home health care services to patients in their homes.
  • Assess and evaluate patient's needs and develop a plan of care accordingly.
  • Administer medications, treatments, and therapies as prescribed by the physician.
  • Provide patient education and counseling.
  • Monitor patient's progress and adjust care plans as needed.
  • Document patient's care and progress in the medical record.
  • Maintain communication with physicians, other healthcare providers, and family members.

Pay: $90,000.00 - $105,000.00 per year
