Role: Ambulatory Triage Nurse - Primary Care supporting multiple locations.
Responsibilities:
Conduct comprehensive patient assessments, develop individualized care plans, perform initial triage to prioritize care, collaborate with healthcare providers for care coordination, provide patient education and emotional support, and maintain accurate documentation and administrative tasks.
Qualifications and
Skills:
Diploma from an accredited nursing program, current Illinois RN license, BLS certification, 1-2 years of clinical experience, and strong communication skills.
