About this role
The Case Manager Registered Nurse (RN) provides comprehensive and coordinated care to patients. Working under moderate or general supervision, this role analyzes patient needs, participates in care coordination, conducts research, reconciles information, and reviews care plans.
Essential Job Function
Analyzes patient cases to assess their needs and to develop individualized care plans. Prepares and processes documentation related to patient care to ensure accuracy and compliance with regulatory requirements. Researches and stays up-to-date with evidence-based practices and healthcare guidelines relevant to the assigned patient population to incorporate the latest knowledge into care plans and interventions. Reviews and evaluates the effectiveness of care plans, monitoring patient outcomes and making adjustments as needed to optimize patient well-being to achieve desired goals. Performs other duties as assigned.
Education
Required: Associate's Degree Preferred: Bachelor's Degree
Experience
Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
Licensure/Certification/Listing
Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
