About this role
Title: Travel Nurse/ LTSS Service Care Manager Travel Requirement: Up to 80% travel for member visits (3–4 days/week in the field) Duration: 6 Months (Potential Extension/Conversion) Training: 3.5 weeks, 8 AM–5 PM EST, camera on; no time off during training.
We are seeking an experienced LTSS Service Care Manager to provide care coordination, assessments, service planning, and member support.

Responsibilities
- Manage LTSS member caseloads and individualized care plans
- Conduct in-person assessments and member visits
- Coordinate healthcare, community, and long-term care services
- Monitor service delivery and address gaps in care
- Collaborate with providers, physicians, and discharge planners
- Educate members and families on benefits and available services
- Maintain accurate documentation and regulatory compliance
Requirements
- Bachelor’s degree required
- RN or LCSW/LCSW-A required
- 2–4+ years of healthcare management/case management experience
- Hospital case management, home health, discharge planning, UM, or LTC experience preferred
- Valid driver’s license required
- Strong communication, documentation, and EMR skills
Pay: $40.00 - $42.00 per hour
