Hiring Immediately! Hospice RN Case Manager. Job Pay based on experience. We are seeking a hospice case manager dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this pivotal role, you will be responsible for coordinating patient care, ensuring that individuals receive comprehensive and effective medical services. You will work closely with patients, families, and healthcare providers to develop and implement personalized care plans that address the unique needs of each patient. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for patient advocacy.
Responsibilities
Assess patients' medical needs and develop individualized care plans in collaboration with the healthcare team. Coordinate and manage patient services in hospice care in their own home. Maintain accurate medical records and documentation to ensure continuity of care. Provide education and support to patients and their families regarding treatment options and health management. Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes. Collaborate with interdisciplinary teams to facilitate effective communication among healthcare providers.
