About this role
We are currently seeking a detail-oriented and experienced MDS Coordinator to join our team!
Responsibilities
Complete and oversee accurate and timely MDS assessments in accordance with federal and state regulations. Coordinate care plans and ensure compliance with Medicare/Medicaid guidelines. Collaborate with interdisciplinary team to optimize resident care and reimbursement. Monitor documentation and ensure accuracy for audits and reviews. Stay up to date with regulatory changes and MDS guidelines.
Qualifications
Active RN or LPN license. Previous MDS experience in LTC/SNF preferred. Strong knowledge of RAI process and care planning. Excellent attention to detail and organizational skills.
