We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Adams Lane Healthcare & Rehabilitation. In this role, you will provide high-quality nursing care to our residents, ensuring their health and well-being while contributing to our mission of family-focused care.

Starting pay for this position, based on experience, is $28.50 - $30.00 an hour for LPNs and $35 an hour for RNs. PLUS an additional $2.50
- Shift differential.
The typical schedule for this opportunity is Three(3) 12-hour shifts a week 7pm - 7am. The standard schedule for this position includes an every other weekend rotation. Specific schedules available will be discussed during interview.

Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Monitor patient health and provide updates to the healthcare team.
- Assist in the development and implementation of individualized care plans.
- Perform assessments, including vital signs and medical history evaluations.
- Educate patients and families on health management and wellness practices.
- Coordinate with interdisciplinary teams to ensure comprehensive care delivery.
- Document patient progress accurately in electronic health records.
Requirements
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Ohio.
- Experience in Long Term Care, Behavioral Health or Acute Care preferred.
- Strong clinical assessment skills and attention to detail.
- Excellent communication and interpersonal skills for effective patient interaction.
- Familiarity with electronic health record systems such as Point Click Care is a plus
