Be Heard: Share your ideas and help shape the way we work through improvement huddles, employee surveys, and town hall meetings Job Description A Day in the Life Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs. Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality cand cost-effective health care services. Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization. Support quality improvement initiatives and adherence to safety standards. Qualifications What’s Required Licensed RN in the State of Michigan Experience in complex chronic disease management preferred Bachelor of Science degree in nursing preferred Five years' (5) experience as an RN preferred Additional Information Are you Munson Material?
