We are seeking an experienced, compassionate, and highly organized Registered Nurse (RN) to join our growing team in a full-time hybrid clinical and care management position.

This is a unique role for an RN who enjoys working with older adults and their families outside of a traditional hospital setting. The RN will divide their time between Flex Care clinical oversight, care management, client assessments, medication management, staff education, and direct billable client services.

Approximately 50% of the RN’s working time is expected to be billable client care and care management services, with the remaining time dedicated to clinical oversight, Flex Care, staff support, documentation, quality assurance, and coordination.

Key Responsibilities

Care Management & Billable Client Services

● Conduct comprehensive client assessments and reassessments.

● Provide Aging Life Care/care management services to clients and families.

● Develop individualized care plans.

● Attend medical appointments when requested.

● Communicate with physicians, pharmacies, hospitals, rehabilitation facilities, assisted living communities, hospice providers, and other healthcare professionals.

● Provide medication reconciliation and medication management.

● Set up weekly medication/pill boxes when ordered and appropriate.

● Monitor changes in clients’ medical, cognitive, functional, and psychosocial conditions.

● Assist families with healthcare decisions, transitions of care, and long-term care planning.

● Provide skilled RN visits and other nursing services within the RN’s scope of practice.

● Maintain timely and accurate documentation of all billable services.

Flex Care

● Provide clinical oversight for the Flex Care program.

● Assess residents who may benefit from Flex Care services.

● Develop appropriate service plans.

● Monitor changes in residents’ needs.

● Coordinate with caregivers, families, facility staff, and other healthcare professionals.

● Provide medication management and other nursing services as appropriate.

● Help identify opportunities for additional services when a client’s needs increase.

Clinical Oversight

● Complete initial assessments, reassessments, supervisory visits, and care-plan updates.

● Develop and review client plans of service.

● Supervise personal care services in accordance with company policies and Virginia requirements.

● Monitor caregiver performance and client outcomes.

● Provide caregiver education, skills training, and competency evaluations.

● Assist with orientation and clinical training of new caregivers.

● Respond to changes in client condition and determine appropriate interventions.

● Participate in quality assurance and clinical compliance activities.

● Ensure documentation meets company and regulatory standards.

Virginia regulations require an RN to be responsible for supervision of personal care services and require written plans of service developed by an RN, including assessment of client needs and required services.

Productivity Expectations

This position combines clinical leadership with revenue-generating client services.

The RN will be expected to

● Maintain approximately 50% billable utilization through care management, assessments, medication management, skilled nursing visits, and other approved client services.

● Accurately document and submit billable time.

● Help grow the company’s care management and Flex Care programs.

● Develop strong relationships with clients, families, facilities, physicians, and community partners.

● Identify changing client needs and communicate appropriate service recommendations.

Qualifications

● Current Registered Nurse license in Virginia or qualifying Compact State license.

● Minimum 2 years of nursing experience preferred.

● Experience with older adults, home care, assisted living, hospice, geriatrics, or care management strongly preferred.

● Strong assessment and clinical judgment skills.

● Excellent communication and documentation skills.

● Comfortable working independently in clients’ homes and senior living communities.

● Strong organizational and time-management abilities.

● Valid driver’s license and reliable transportation.

● Ability to travel throughout Northern Virginia.

● Comfortable using electronic documentation and scheduling systems.

● Experience with WellSky Personal Care is a plus.

The ideal candidate is an RN who wants more than a traditional bedside nursing position. This person enjoys building long-term relationships with older adults and families, solving problems, coordinating complex care, educating caregivers, and helping clients remain safe and independent.

Full-Time Registered Nurse – Care Management & Flex Care

Schedule

Full-time, primarily Monday–Friday, with flexibility based on client needs.

This is an excellent opportunity for an experienced RN interested in a leadership-oriented position that combines nursing, geriatric care management, Flex Care, client advocacy, and program growth.

Pay: $92,000.00 - $135,000.00 per year
