Parham Healthcare & Rehabilitation Center is seeking a skilled and compassionate Registered Nurse Assessment Coordinator (MDS/RNAC) to join our team in Henrico, VA. As a leader in short-term and long-term skilled rehabilitation, we provide exceptional care to our patients and residents.

Job

Summary

The ideal candidate will be responsible for processing related to the assessment of patients and residents in our healthcare facility. The RNAC assesses patients, provides regular reports to doctors and other medical professionals, and works with the MDS system to ensure the best possible care.

Responsibilities

Implements and interprets organizational policies and procedures, and nursing standards and regulations

Implement and interpret MDS (Minimum Data Set) assessments

Review and revise care plans as needed
