Primary Care Nurse Practitioner (Family or Adult)

Resolution Health and Wellness Center is seeking a compassionate and patient-focused Family Nurse Practitioner (FNP) to join our integrated outpatient clinic serving the Baltimore area. This role is ideal for a provider who values whole-person care, clinical autonomy, and collaboration within a multidisciplinary team.

We specialize in delivering high-quality, integrated healthcare to patients with mental health conditions, substance use disorders, behavioral health needs, and chronic medical conditions. Our approach emphasizes patient-centered care, strong clinical outcomes, and an exceptional patient experience.

Position Overview

The Nurse Practitioner will provide comprehensive primary care services, including evaluation, diagnosis, and ongoing management of patients in a collaborative behavioral health setting.

Key Responsibilities

Conduct initial medical evaluations and ongoing patient follow-ups Assess, diagnose, and manage acute and chronic conditions Order and interpret diagnostic tests, labs, and imaging Develop and oversee treatment plans tailored to individual patient needs Maintain timely and accurate clinical documentation Coordinate and refer patients to in-house and external services as appropriate Collaborate with behavioral health providers and clinical staff to ensure integrated, team-based care Qualifications

  • Board-certified Family Nurse Practitioner ((FNP)) or Physician Assistant (PA-C)
  • Active DEA license and BLS/CPR certification
  • Strong clinical judgment, communication skills, and bedside manner
