Licensed Practical Nurse

• Provide skilled nursing care to clients in their homes in accordance with the Plan of Care.
• Monitor and assess clients' physical condition and report changes to the Registered Nurse (RN), Director of Nursing, physician, and family as appropriate.
• Administer medications and treatments as prescribed by the physician and within the LPN scope of practice.
• Perform routine nursing procedures including wound care, dressing changes, catheter care, ostomy care, and vital sign monitoring.
• Observe, document, and report patient responses to treatments and interventions.
• Educate clients and family members regarding disease management, medications, safety precautions, and home care needs.
• Maintain accurate and timely clinical documentation in accordance with agency policies and state regulations.
• Collaborate with the RN Supervisor, physicians, therapists, Home Health Aides (HHAs), and other members of the healthcare team.
• Assist in developing and implementing individualized care plans.
• Promote client independence, dignity, comfort, and quality of life.
• Ensure compliance with infection control, safety, HIPAA, and agency policies.

Qualifications

• Current and active New Jersey Licensed Practical Nurse (LPN) license in good standing.
• Graduate of an accredited Practical Nursing program.
• Minimum one (1) year of nursing experience preferred; home care experience is a plus.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to travel to clients' homes throughout Bergen County and surrounding areas.
• Knowledge of nursing practices, medication administration, wound care, and patient assessment.
• Ability to accurately document patient care using electronic or paper records.
• Strong communication, organizational, and time-management skills.
• Ability to work independently and collaboratively with physicians, RNs, HHAs, patients, and family members.
• Ability to maintain patient confidentiality and comply with HIPAA regulations.
• Must pass background check, reference verification, and health screening requirements.

Pay: $52.00 per hour
