Registered Nurse - Community Outreach

Role Overview: The Registered Nurse provides comprehensive nursing care during patients' hospital stays, assessing, managing, and coordinating treatment plans in collaboration with the healthcare team. Key Responsibilities Conducts thorough patient assessments, including admission, transfer, post-op, and discharge needs, documenting findings accurately. Develops and evaluates individualized care plans based on assessments, diagnoses, lab data, and protocols, setting short and long-term goals. Performs and documents nursing interventions, administers medications, monitors IV fluids, and facilitates referrals and consultations. Provides patient and family education, discharge planning, and involves families in decision-making. Recognizes and responds to changes in patient conditions, manages specialized care such as restraints, pain, and abuse assessments. Ensures adherence to safety, infection control, and quality standards, maintaining a safe environment and practicing proper precautions. Communicates effectively with patients, families, and colleagues, delivering respectful, compassionate service and thorough handoffs. Participates in performance improvement, evidence-based practice, research, and departmental initiatives. Delivers age-appropriate care for adult and geriatric patients, optimizing resource use and ensuring cost-effective services. Qualifications & Skills Current RN license in applicable states and BLS certification. Technical proficiency in nursing procedures, documentation, and patient safety practices. Strong communication, teamwork, and professionalism skills. Ability to adapt to changing demands and work in a fast-paced environment. Commitment to quality care, safety, infection control, and ethical standards. Additional Duties: Supports a positive work environment, mentors new staff, and upholds hospital policies and standards to ensure excellent patient care and service excellence.
