*Job Description*
We are seeking a *Physician Assistant (PA)* or *Nurse Practitioner (NP)* to join our rehab team as a part-time independent contractor. You will round on patients undergoing physical, occupational, or speech therapy in Skilled Nursing Facilities (SNFs), supporting both clinical care and facility rehab metrics.
*Key Responsibilities*
- Conduct *2 patient visits per week per facility*
- *1st visit* during *weekday business hours* (to collaborate with SNF staff)
- *2nd visit* flexible (evenings/weekends acceptable)
- Monitor progress toward rehab goals
- Collaborate with therapy teams and SNF staff to go over patient caseload and provider clinical recommendations.
- Use *remote scribe service* for most documentation
- Occasionally cover other providers when they are away (volume may increase)
*Requirements*
- Valid *PA License* or *NP Certification* (Required)
- Ability and *willingness to drive* to all assigned facilities
- *Weekend availability needed*
- Strong communication and organizational skills
- Comfortable with basic tech: *MS Office / Google Drive*
- Willing to learn SNF workflows and rehab metrics (training provided remotely)
