Provides skilled nursing care to clients in their homes or community health settings ensuring the delivery of outcome based, high quality, cost effective care.

Requirements

  • RN License (state of CT)
  • Associate's Degree or diploma program in nursing (required)
  • BSN (preferred)
  • 1 year nursing experience (required)
  • Medical/surgery or home care experience (preferred)
  • Computer skills
  • OASIS assessment experience
  • In-home healthcare experience
