*Job Overview* We are seeking a compassionate and dedicated Certified Home Health Aide to join our team in providing exceptional in-home care to individuals with diverse needs. As a vital member of our healthcare team, you will deliver personalized support that promotes independence, safety, and well-being. Your role will involve working closely with patients, families, and healthcare professionals to ensure comprehensive and respectful care in the comfort of their homes. *Duties*

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support, using tools such as Hoyer lifts and other assistive devices.
  • Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; document findings accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance.
  • Administer medications as prescribed, ensuring proper documentation and adherence to medical protocols.
  • Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral support strategies.
  • Support patients with complex medical needs such as catheter care, wound management, hospice care, and working with individuals with developmental disabilities or disabilities.
  • Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met.
  • Document all patient interactions thoroughly in medical records or EMR systems; communicate effectively with healthcare teams regarding patient status or changes.
  • Assist with medical procedures under supervision when required and ensure compliance with all safety guidelines during patient care activities.
  • Engage in ongoing training on medical terminology, CPR certification renewal, and best practices in home health care to enhance your caregiving skills.

*Experience*

  • Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred.
  • Demonstrated ability to work effectively with seniors and individuals requiring long-term or hospice care.
  • Experience caring for patients with dementia, Alzheimer's disease, or other cognitive impairments is desirable.
  • Familiarity with Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems for documentation purposes.
  • Knowledge of medical terminology and procedures related to home health services.
  • Proven ability to handle sensitive situations with compassion while maintaining professionalism and confidentiality under HIPAA guidelines.
  • Previous experience working with individuals who have disabilities or developmental challenges is a plus.
