*Roles & Responsibilities*
- Provide direct nursing care according to individual patient/resident care plans
- Administer medications and treatments safely and accurately
- Monitor, assess, and document patient/resident conditions
- Assist with wound care, treatments, and coordination of nursing tasks
- Communicate effectively with RNs, physicians, families, and facility staff
- Maintain accurate documentation and follow facility, state, and federal guidelines
- Promote a safe and high-quality care environment
*Requirements*
- *Active LPN license in CO/Compact License*
- Minimum 1 year of LPN experience (LTC/SNF preferred)
- Must be reliable, professional, and able to work consistently
- Strong clinical judgment and communication skills
