TeamAvame A Registered Nurse (RN) in a memory care and assisted living facility acts as both a clinical leader and direct caregiver. They manage individualized care plans, assess cognitive and physical changes, supervise care staff, and guide dementia-specific interventions to maintain residents' safety, dignity, and quality of life. As a Registered Nurse, you will play a key role in providing leadership and mentorship to care staff, ensuring that the care we provide aligns with state regulations and organizational policies. The ideal candidate will possess excellent clinical skills, strong leadership abilities, and a passion for working with seniors in a long-term care setting. We are seeking on-call to assist with Assessments on Fridays.

Resident Assessments: Conduct thorough assessments of new and existing residents to identify healthcare needs, preferences, and goals. Continuously update care plans based on changes in health status. Care Planning & Documentation: Develop, implement, and evaluate individualized, person-centered care plans in collaboration with residents, families, and interdisciplinary team members. Maintain accurate and up-to-date nursing documentation in compliance with state regulations and facility policies Medication Management: Oversee medication administration, reconcile dosages, monitor for side effects, and manage electronic medication administration records Training & Mentorship: Educate resident assistants and medication technicians on dementia-specific best practices, infection control, and emergency procedures. Task Delegation: Delegate nursing tasks (under state-specific nursing practice acts) and supervise direct care staff to ensure high standards are met. Daily Oversight: Ensure staff are properly following structured daily routines, which are critical for lowering stress and supporting memory care residents. Family Communication: Act as the primary point of contact for families regarding health changes, care adjustments, and progression of the disease. Advocacy: Provide emotional and psychosocial support to residents and their families, connecting them to community resources. Problem-Solving & Critical Thinking: Handle dynamic and evolving healthcare situations by applying critical thinking skills and effectively responding to emergencies both in person and remotely, addressing changes in resident health, and staff challenges Medical Coordination: Coordinate services with external providers, including physicians, hospice teams, pharmacies, and physical therapists. Other duties assigned are appropriate to the position Requirements Must possess a nursing degree from an accredited college or university Must possess a current, unencumbered, active license to practice as a Registered Nurse / RN in this state. BSN preferred Nursing experience in assisted living, Alzheimer/dementia, long term care, geriatric rehab, sub-acute and or critical care clinical setting Thorough knowledge of CMS and State Regulations regarding patient care in an ALF/IL/MC facility. Experience in electronic medical records documentation. Strong leadership and communication skills. Ability to design and deliver effective training programs Knowledge of State and Federal regulations and compliance requirements Excellent organizational and time management skills. Must have and maintain an active CPR/BLS during employment.
