About this role

*Nurse Practitioner*

*Location:* Elmira, NY

  • *Hybrid*
  • *In-person: 3-4 days per week*, *7:30 AM – 11:30 AM* (12 hours/week - onsite)
  • Remaining hours will be remote
  • (20 to 40 hrs. per week)

We are seeking a compassionate and experienced Nurse Practitioner to provide direct patient care in an alcoholism and substance use treatment setting. This role is responsible for patient assessment, diagnosis, and collaboration in developing and implementing individualized health plans.

*Essential Responsibilities*

  • Maintain a professional, welcoming, and supportive presence with all patients
  • Conduct comprehensive physical and psychological assessments, including health histories, physical exams, and diagnostic testing
  • Diagnose medical conditions and initiate appropriate treatment plans aligned with patient needs
  • Ensure patient comfort, safety, privacy, and confidentiality at all times
  • Develop and maintain detailed health profiles using clinical, diagnostic, and psychosocial data
  • Identify and evaluate normal and abnormal clinical findings
  • Provide initial and ongoing assessments to determine diagnosis and appropriate level of care
  • Review and sign client treatment plans and medical documentation as required
  • Review medical assessments and physical exams from outside providers when applicable
  • Provide emergency medical care in life-threatening situations
  • Coordinate disease prevention and health maintenance initiatives
  • Collaborate with physicians and interdisciplinary team members to deliver high-quality care
  • Provide patient education materials and follow-up instructions
  • Design and implement educational programs to support clinical and community needs
  • Maintain compliance with NYS Public Health Law, NYS OASAS regulations, 42 CFR, HIV regulations, and all confidentiality requirements
  • Complete accurate and timely medical documentation using agency medical software
  • Participate in case conferences, supervision, and weekly interdisciplinary meetings
  • Meet productivity standards and actively engage clients to reduce missed appointments
  • Maintain Qualified Health Professional (QHP) status
  • Perform other related duties as assigned

*Knowledge, Skills & Abilities*

  • Advanced understanding of substance use disorders and their impact on individuals and families
  • Knowledge of community and treatment resources for substance use disorders
  • Ability to recognize symptoms of substance use and identify contributing factors
  • Strong documentation skills and proficiency with electronic medical records and computer applications
  • Excellent verbal and written communication skills
  • Ability to build and maintain effective working relationships with patients, staff, and community partners

*Qualifications*

  • New York State Nurse Practitioner license
  • Current New York State Registered Nurse license
  • DEA X Waiver (MAT required)
  • Prior clinical experience in a substance use treatment setting
  • Valid driver’s license
