About this role

Hospice RN / RN Case Manager role delivering personalized hospice and palliative care to patients facing life-limiting illness. Primary point of contact for patients and families, delivering hands-on nursing care based on individualized Plan of Care in home, assisted living, or other settings. Perform initial and ongoing assessments, collaborate with interdisciplinary team, provide emotional and spiritual support, teach caregivers, maintain clinical documentation, and support quality improvement initiatives.
