About this role

We are seeking a compassionate and experienced Part-Time Hospice Field Registered Nurse (RN) to provide patient-centered care to individuals with life-limiting illnesses in their homes and other care settings.

The Hospice Field RN develops, coordinates, and manages individualized plans of care that address each patient’s physical, functional, emotional, psychosocial, cultural, and spiritual needs. Working closely with physicians, hospice aides, social workers, chaplains, and other members of the interdisciplinary group (IDG), the RN helps ensure comfort, dignity, and quality of life for patients while providing education and support to their families and caregivers.

Key Responsibilities Conduct initial and ongoing comprehensive assessments of the impact of terminal illness on each patient’s physical, functional, psychosocial, environmental, cultural, and spiritual needs. Assess activities of daily living, caregiver capabilities, grief risks, and verbal and nonverbal indicators of patient and family needs. Apply established admission and recertification criteria to determine hospice eligibility and the appropriate level of care.

Develop, implement, document, and regularly update individualized plans of care to promote quality and continuity of services. Determine the appropriate scope and frequency of nursing and support services based on patient acuity and patient and family needs. Evaluate the caregiver’s ability to meet the patient’s immediate and ongoing needs throughout the course of care.

Educate patients and families regarding the disease process, symptom management, self-care techniques, end-of-life care, nutrition, and dietary needs. Communicate and prioritize patient and family needs with members of the interdisciplinary group. Provide clinical direction and supervision to Hospice Aides and LPNs/LVNs to ensure high-quality, coordinated care.

Prepare accurate and timely clinical documentation and progress notes demonstrating progress toward established goals. Notify physicians and other appropriate team members of changes in the patient’s condition, needs, or response to interventions. Evaluate patient and family responses to care and recommend revisions to the plan of care as needed.

Participate in case conferences, team meetings, evaluations, and care coordination activities. Provide education and clinical training to staff as needed. Perform on-call responsibilities and provide support to patients and families as assigned.
