LVN/RN - Friendly and Supportive Skilled Nursing Team

Responsibilities
Provide direct patient care, including administering medications, managing IV lines, and performing wound care
Assist with patient assessments, develop care plans, and implement interventions
Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
Educate patients and their families on self-care techniques and disease management strategies
Maintain accurate and timely documentation of patient records and care plans
