Ready to make a meaningful impact from the very first patient touchpoint? Singlepoint Healthcare is looking for a Registered Nurse Intake Nurse Specialist to help coordinate safe, smooth, and compassionate home infusion care for patients.

What You’ll Do

  • Assess patients for home infusion therapy
  • Coordinate care plans, supplies, and delivery schedules
  • Partner with home health agencies, referral sources, and pharmacy teams
  • Support patient and caregiver education
  • Monitor progress, therapy compliance, and care needs
  • Keep clinical documentation accurate and timely

What You’ll Bring

  • RN license in the state of practice
  • Associate’s degree required, Bachelor's preferred (BSN).
  • At least 1 year of nursing experience in home health or infusion therapy is preferred or 2-5 years of nursing experience
  • Knowledge of infusion techniques, home care protocols, and patient education
  • Strong communication, organization, and patient-first mindset
