About this role

Gentle Care Home Services is seeking compassionate and dependable Licensed Practical Nurses (LPNs) interested in providing personalized, one-on-one nursing care to children in their homes. Our patient needs and schedules vary, allowing our recruiting team to help identify available cases that may be a good match for your clinical experience and availability.
Responsibilities
- Provide one-on-one nursing care according to the patient's plan of care.
- Monitor and report changes in the patient's condition.
- Administer medications and prescribed treatments.
- Provide G-tube, tracheostomy, ventilator, seizure, and other skilled care when applicable.
- Assist with activities of daily living as appropriate.
- Complete accurate and timely documentation.
- Communicate with families and the Gentle Care clinical team.
- Provide compassionate support to patients and families.
Requirements
Qualifications
- Active, unrestricted Georgia Licensed Practical Nurse (LPN) license.
- Current CPR certification.
- Ability to meet required background and health screening requirements.
- Pediatric or home health experience preferred.
- Clinical skill requirements vary by patient assignment.
