About this role
Functions as a clinical expert and care coordinator for a specific patient population at the University of Iowa Health Care Department of Nursing, Medicine Specialty Clinic. Collaborates with nursing, medical staff and other health professionals in developing and reviewing standards and in planning, implementing and evaluating patient care. Independently coordinates and facilitates patient care between multiple disciplines within UI Health Care and local communities.
Coordinates patient care through patient visits, pre-scheduling testing, follow-up with test results, obtaining medical history, coordinating follow-up appointments, and providing medication and rehab teaching. Position is 60%-100% (24-40 hours per week), variable days Monday-Friday with no weekend, holiday, or call coverage.
