What You'll Do

As a Licensed Practical Nurse, you'll play an important role in providing quality care and supporting positive patient outcomes.

Responsibilities include

• Provide direct patient care and monitor resident conditions
• Administer medications and treatments as ordered
• Monitor vital signs and report changes in condition
• Assist with assessments and care planning
• Document care accurately in electronic medical records
• Collaborate with physicians, RNs, and interdisciplinary care teams
• Provide wound care and nursing procedures within scope of practice
• Educate residents, patients, and families on care plans and treatments
• Promote a safe, compassionate, and patient-centered environment

Required Qualifications

A Nursing degree from an accredited college or university. Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state. Current CPR certification. Prior experience in long-term care and/or home health and hospice is preferred but not required. Food Handler Permit as required
