About this role
Join Pilgrim Place, a mission-driven senior living community in Claremont, where compassion, teamwork, and person-centered care guide all we do. We are seeking a dedicated Neighborhood/Household Charge Nurse (LVN) (PM Shift) to join our team in a on-call role.
In this role, you’ll provide hands-on nursing care, support a resident-directed culture, and serve as a teacher and role model for neighborhood/household staff, ensuring high-quality, individualized care for every resident.

Administer medications and treatments safely and according to plan of care.

Observe residents’ physical and emotional condition and report changes to the Nurse Supervisor (RN), Director of Nursing, or attending physician.
Collaborate closely with the Nurse Leader to ensure regulatory compliance and best practices.
Support a person-centered, resident-directed environment that fosters independence and choice.
Serve as a mentor and role model for neighborhood/household staff, promoting professional nursing practices.
Provide routine daily nursing care and services aligned with each resident’s plan of care and personal preferences.
Advocate for residents’ needs and contribute to a safe, supportive, and homelike environment.

What You Bring
Graduate of an accredited school of professional nursing.
Current, active California Licensed Vocational Nurse (LVN) license.
Minimum of one year of experience in a skilled nursing or senior living setting preferred.
Ability to read, write, and communicate effectively in English.
Strong clinical judgment, observation, and documentation skills.
Commitment to fostering a resident-centered, collaborative, and learning-focused team environment.
