About this role
*Job Summary* We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join Embracing Hearts Healthcare. Embracing Hearts Healthcare provides services through the MS IDD Waiver, which is a Medicaid waiver for individuals with Intellectual and Developmental Disabilities. We go *in home* and provide nursing respite services. The ideal candidate will possess a strong background in patient care, with experience in home care. As an LPN, you will play a vital role in providing high-quality nursing care to patients, assisting with their daily needs, and collaborating with healthcare professionals to ensure optimal patient outcomes.

- Provide direct patient care.
- Administer medications and Feedings.
- Conduct patient assessments, monitor vital signs, and document medical histories.
- Assist in the development and implementation of individualized care plans.
- Perform tube feeding, and other nursing procedures as needed.
- Support patients with activities of daily living (ADLs) while promoting independence.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Educate patients and families on health management, medication administration, and disease prevention.
- Ensure a safe and clean environment for patients by adhering to infection control protocols.
*Requirements*
- Valid LPN license in the state of practice.
- Experience in Home care.
- Strong assessment skills and ability to perform basic life support (BLS).
- Experience working with individuals with developmental disabilities or dementia care is desirable.
- Excellent communication skills and ability to work collaboratively within a team environment.
