Admissions Registered Nurse

• Conducts comprehensive hospice admission assessments including physical, functional, psychosocial, and spiritual assessment of the patient and family/caregiver in the home or facility setting.
• Verifies Medicare hospice eligibility: confirms terminal diagnosis, six-month prognosis consistent with the primary diagnosis and LCD criteria, and physician certification of terminal illness.
• Facilitates the hospice election process: presents the Notice of Election (NOE) to the patient/family, explains the hospice benefit and waiver of curative treatment, and ensures the election statement is completed and signed.
• Completes all required hospice admission documentation within required timeframes: election statement, initial assessment, plan of care, patient/family rights, and advance directive review.
• Assesses patient/family goals of care, comfort preferences, and psychosocial needs at admission; communicates findings to the IDG and Case Manager.
• Coordinates with the attending physician to confirm hospice certification documentation requirements are initiated at the time of admission.
• Ensures the comfort kit (emergency medication kit) is ordered, dispensed, and that the patient/family receives education on its use at or immediately following admission

*Skills*

• Registered Nurse (RN) with current, unrestricted licensure to practice professional nursing in the State of Arizona required.
• Minimum of two (2) years of clinical nursing experience required; home health and/or hospice experience strongly preferred.
• Working knowledge of OASIS-E2 (effective 04/01/2026) and Medicare home health documentation requirements required.
• Working knowledge of Medicare hospice eligibility criteria, CMS Conditions of Participation (42 CFR §418), hospice election requirements, and levels of care required.
• Working knowledge of ICD-10-CM coding and PDGM clinical grouping for home health episodes preferred.
• Demonstrates excellent clinical assessment, observation, problem-solving, verbal and written communication skills.
• Self-directed and able to work with minimal supervision; demonstrates strong organizational and time management skills.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile insured in accordance with state and/or organization requirements and in good working order.
• Demonstrates ability to organize and prioritize workload independently in a fast-paced, high-volume admissions environment..
