Perform comprehensive nursing assessments and develop, implement, and evaluate individualized plans of care that promote comfort, dignity, and quality of life. Administer medications, IV therapies, wound care, and other skilled nursing interventions while continuously monitoring patient response and adjusting care as appropriate. Assess changes in patient condition, communicate clinical findings promptly, and collaborate with physicians and the interdisciplinary team to ensure timely, coordinated care. Educate patients and families on hospice philosophy, disease progression, medications, pain and symptom management, and available support resources, providing compassionate guidance throughout the hospice journey. Coordinate admissions, transfers, discharges, and care transitions to ensure seamless continuity of care for patients and families. Accurately document assessments, interventions, medication administration, and patient outcomes within the electronic medical record in accordance with regulatory and organizational standards. Collaborate with physicians, advanced practice providers, social workers, chaplains, pharmacists, Hospice Aides, and other interdisciplinary team members to deliver exceptional hospice care. Serve as a clinical leader by mentoring and supporting fellow caregivers, promoting teamwork, and contributing to quality improvement and professional development initiatives. Maintain compliance with Medicare Conditions of Participation, state regulations, accreditation standards, and organizational policies while providing safe, high-quality patient care. Qualifications Current, unrestricted Registered Nurse (RN) license. Minimum of one (1) year of RN experience in a medical-surgical, acute care, or related clinical setting required. Hospice experience preferred (or required where state regulations apply). Bachelor's degree in Nursing preferred. Current CPR certification if required by state regulations. Strong clinical assessment, critical thinking, and pain and symptom management skills. Excellent communication and interpersonal skills with a compassionate, patient-centered approach to care. Experience using electronic medical records and clinical documentation systems preferred. $31.00 - $36.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position.
