About this role
We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our skilled nursing facility team. The ideal candidate will possess a strong background in patient care, with experience in long-term care, acute care, and specialized nursing practices. In this role, you will provide comprehensive nursing services to residents, ensuring their health, safety, and well-being are prioritized through expert assessment, medication administration, and personalized care planning. - Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to inform individualized care plans.
- Administer medications and treatments accurately according to physician orders while adhering to safety protocols and HIPAA regulations.

- Monitor residents’ conditions closely, recognizing changes that require intervention or escalation to medical teams.
- Collaborate with interdisciplinary teams—including physicians, therapists, and social workers—to coordinate comprehensive care strategies.
- Provide education and support to residents and their families regarding health conditions, medication management, and post-discharge instructions.
- Assist with procedures such as IV infusions, catheterization, wound care, and other advanced nursing interventions as required.
- Ensure infection control practices are rigorously followed to maintain a safe environment for residents and staff.
Skills
- Proven experience working in skilled nursing facilities with familiarity in long-term care regulations and compliance standards.
- Strong clinical skills including patient assessment, medication administration, IV infusion, and wound management.
- Experience with specialized care areas such as Alzheimer’s/dementia care, memory care, hospice services, and behavioral health needs.
- Ability to perform triage effectively in urgent situations while maintaining calmness under pressure.
- Excellent communication skills for caregiver collaboration and client interaction across diverse populations including geriatric patients and individuals with disabilities.
- Knowledge of infection control protocols, HIPAA compliance policies, and physical examination techniques.
