WHAT YOU'LL DO (Your Impact): Master the MDS Process: Coordinate and oversee the timely completion of all MDS assessments (OBRA and PPS) from admission to discharge, ensuring 100% accuracy and compliance with Pennsylvania state and federal guidelines. Collaborate with the Interdisciplinary Team (IDT): Work closely with nursing, therapy, dietary, and social services to build truly person-centered care plans across our campus that accurately reflect resident needs. Maximize Clinical Reimbursement: Review clinical documentation with a sharp eye to ensure it fully supports the care provided, capturing the appropriate resource utilization data for accurate reimbursement. Audit & Support: Conduct routine quality reviews on medical records and provide helpful feedback to floor staff regarding the vital importance of complete, precise clinical documentation. Manage Transmission Data: Assist in ensuring the smooth, error-free electronic submission of all MDS data to CMS, executing course corrections whenever needed. WHAT YOU BRING TO THE TEAM: The Clinical Standard: An active, unencumbered Licensed Practical Nurse (LPN) license in the state of Pennsylvania. (Must be an LPN to apply). MDS/RAI Experience: Hands-on experience managing or assisting with the MDS / RAI process in a skilled nursing or long-term care setting. Certification: RAC-CT (Resident Assessment Coordinator Certified) designation or LPNAC equivalent is a massive plus, but a proven track record of clinical coding excellence is highly valued!
