Requirements
- Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Pennsylvania
- Hands-on experience managing or assisting with the MDS / RAI process in a skilled nursing or long-term care setting
- RAC-CT (Resident Assessment Coordinator Certified) designation or LPNAC equivalent preferred
- Meticulous attention to detail and analytical mindset
- Exceptional organizational skills
- Ability to collaborate across different departments
- Communication skills
- Skilled nursing facility experience
