- Perform comprehensive patient assessments, including vital signs, intake, and physical examinations to determine immediate needs.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration.
- Document all patient interactions thoroughly using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems like PCC, ensuring compliance with HIPAA regulations.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans for patients with complex conditions such as dementia, Alzheimer's disease.
*Experience*
- Valid LPN license with current state licensure required.
- Demonstrated proficiency with EMR/EHR systems like PCC.
- Background working with individuals with disabilities or those requiring specialized memory or dementia care is desirable.
- Strong understanding of physiology, anatomy, infection control practices, and emergency response protocols such as basic life support (BLS).
