Position
We are currently seeking a Registered Nurse to service San Luis Obispo and surrounding areas:
- Acute care experience preferred
- Hourly and Paid Per Visit (PPV) hybrid model (negotiable based on experience, skill level, availability, etc.)
- Opportunities to accumulate overtime
Patient Care
- Completes initial and ongoing comprehensive assessments of patients and families to determine hospice needs
- Provides professional nursing care using the nursing process and according to the state Nurse Practice Act
- Assesses and evaluates patient status by:
- Initiating and updating plan of care
- Re-evaluating patient and caregiver needs
- Revising care plans as needed
- Develops care plans with patient and family involvement
- Administers medications and treatments as prescribed
- Provides education, counseling, and support to patients and families
- Assists with activities of daily living and promotes comfort and independence
- Acts as Case Manager when assigned
Communication
- Maintains accurate clinical documentation
- Communicates with physicians regarding patient needs and condition changes
- Coordinates care with community health professionals
- Educates patients and caregivers on self-care and treatment plans
- Maintains a safe patient environment
- Participates in interdisciplinary team collaboration and meetings
Additional Duties
- Participates in on-call duties as defined by the on-call policy
- Ensures that arrangements for equipment and other necessary items and services are available
- Supervises ancillary personnel and delegates responsibilities when required
- Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through continuing education and in-service classes
- Fulfills the obligation of requested and/or accepted case assignments
- Actively participates in quality assessment and performance improvement activities
Qualifications
- Graduate of a National League of Nursing accredited school of nursing
- Current California license and current CPR certification
- Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years OR (b) a baccalaureate degree in nursing from an accredited program and a current RN license. A minimum of two years’ experience, with at least one year in public health, home care, or hospice nursing, is preferred
- Must be a licensed driver with an insured, reliable automobile in good working condition
- Management experience not required; responsible for supervising hospice aides
- Self-directed and able to work with minimal supervision
- Excellent observation, verbal and written communication, problem-solving, and basic math skills; nursing skills per competency checklist
- Ability to meet physical demands of the role, including prolonged walking/standing, lifting, bending, kneeling, and adequate visual/hearing acuity
