*Job Overview* Join our dynamic public health team as a Public Health Nurse II, where you will play a vital role in promoting community wellness and disease prevention.   • Provides home visits to women and their families eligible for the NFP Program.
  • Adheres to the Nursing Process and the NFP model of home visitation.
  • A full-time Nurse Home Visitor carries a caseload of 25 and no more than 30 clients.
  • The Nurse Home Visitor applies three theories integral to implementation of the NFP model. The theories that provide a framework for practice in the NFP include the following: Human Ecology, Attachment Theory and Self-Efficacy Theory.
  • The Nurse Home Visitor utilizes six domains or subject areas that are covered with mothers in guiding the comprehensive assessment during each home visit. These domains include: Personal Health, Environmental Health, Life Course Development, Maternal Role, Family and Friends and Health and Human Services.

*Experience*

Prefer (not required) one year of experience in Public Health Nursing.

Pay: $29.27 - $41.41 per hour
