About this role

The purpose of a Registered Nurse at Oak Street Health is to build strong relationships with Oak Street Health patients by coordinating their care and providing a seamless experience to patients and their support team. Core responsibilities include providing competent nursing care, responding to telephonic requests, conducting triage and symptom management, utilizing standardized protocols for medication management, conducting medication reviews, providing patient education particularly around chronic conditions, collaborating on care plans, building relationships with patients and caregivers, delegating activities to clinical care team members, participating in care team meetings, performing point of care testing and specimen collection including phlebotomy.

Requirements

Active Registered Nurse (RN) Licensure in good standing with the applicable state
BLS Certification
Electronic Medical Record (EMR) experience
US work authorization
Ability to maintain patient confidentiality
Ability to assess patients without face-to-face interaction
Strong communication and assessment skills
2+ years of healthcare experience working as an RN (preferred)
