About this role

Seeking an experienced in-office RN Case Manager to play a key leadership role in ensuring high-quality, patient-centered care across the home health continuum. Responsible for coordinating care through established Plans of Care and physician orders while optimizing resource utilization, regulatory compliance, and clinical outcomes. The RN Case Manager serves as a clinical resource, patient advocate, and operational leader across assigned regions through in-person, remote, and telephonic care coordination.

Key responsibilities include collecting and reviewing SOC, Recertification, Evals and Re-Eval reports from clinicians; developing, reviewing, and modifying Plans of Care and physician orders; tracking hospitalizations, falls, infections, and incident reports; advocating for patients; providing supervision and clinical guidance to staff; collaborating with nursing, therapy, and interdisciplinary teams.
