- Active Illinois Registered Nurse (RN) license in good standing
- Minimum three (3) years of recent Medicare-certified home health experience
- Extensive OASIS-E knowledge and experience
- Advanced proficiency with Axxess Home Health software
- Strong understanding of Medicare Conditions of Participation and home health regulatory requirements
Position Summary
Luxe Home Health is seeking an experienced Home Health Quality Assurance RN to support our Illinois operations. This is a contract, primarily remote position responsible for reviewing clinical documentation, auditing OASIS assessments, ensuring regulatory compliance, and supporting quality improvement initiatives.
The ideal candidate is a highly skilled home health clinician who can work independently, identify documentation deficiencies, recognize compliance risks, and provide meaningful recommendations to improve clinical quality and patient outcomes.
While this position is primarily remote, the selected candidate must be available to attend in-person meetings at our Des Plaines office one to two times per week to review workload, discuss findings, and collaborate with lea
