About this role

Position Overview

The Registered Nurse Case Manager provides professional clinical nursing care while coordinating appropriate and timely services for patients. The position focuses on achieving quality and cost-effective healthcare outcomes through patient advocacy, preventive care, chronic disease management, health education, and coordinated case management.

Working as part of an interdisciplinary care team, the Registered Nurse Case Manager helps ensure patients receive comprehensive and individualized care while promoting independence, self-management, and long-term health.

Key Responsibilities

Collaborate with medical providers and interdisciplinary team members to coordinate individualized patient care. Participate in daily team-based pre-visit planning. Proactively manage an assigned patient panel to identify acute and chronic care needs.

Use the electronic health record and population health software to monitor patient needs and improve outcomes. Provide nursing assessments, documentation, patient education, medications, and appropriate treatments. Advocate for patients and families while encouraging participation in healthcare decisions and self-management.

Educate patients and families about procedures, treatments, medications, and preventive health practices. Review inpatient and emergency room admissions and coordinate appropriate follow-up care. Support medication ordering and administration according to KANA policies, protocols, and nursing scope of practice.

Provide education and outreach for obstetric patients and coordinate comprehensive care with primary care providers. Track postpartum care and newborn immunizations. Ensure patient immunizations are current as needed.

Respond appropriately to emergency situations and changes in patient condition. Maintain accurate clinical documentation and follow established nursing standards and KANA protocols. Maintain a clean, safe, and well-equipped clinical environment.

Participate in quality improvement activities and ensure required quality measures are captured and achieved. Support colleagues and contribute to positive, collaborative nursing and clinic team relationships.

Qualifications

Required

Graduation from a two-year or four-year registered nursing program. Minimum of two years of clinical experience in an ambulatory or outpatient care clinic setting. Current Registered Nurse license. Current BLS and CPR certifications. Strong clinical judgment and critical-thinking skills. Ability to communicate effectively with patients, families, providers, coworkers, and community members. Experience using electronic health records and other computer-based healthcare systems.

Preferred

Experience with case management and population health. Experience in preventive health and chronic disease management. Experience working within an interdisciplinary healthcare team. Experience participating in quality improvement initiatives.
