What You’ll Do
- Perform in-home patient assessments
- Develop and follow individualized care plans
- Provide skilled nursing care (wound care, meds, catheter care, chronic disease management)
- Educate patients and caregivers on home care and safety
- Monitor patient progress and update care plans
- Complete timely and accurate documentation
What We’re Looking For
- Active Texas RN license (required)
- Home health or acute care experience preferred
- Strong clinical judgment and adaptability
- Excellent communication and patient care skills
- Reliable transportation & valid driver’s license
- Kinnser/WellSky experience (preferred)
