Position Summary
The Hospice RN Case Manager – Homecare provides compassionate hospice care to patients in their homes and care facilities. This role is responsible for:
- Conducting comprehensive nursing assessments
- Developing and implementing individualized plans of care
- Maintaining the highest level of patient comfort and quality of life
- Coordinating care through an interdisciplinary team approach
- Performing ongoing reassessments and revising care plans as patient/family needs evolve
- Supervising and collaborating with team members to ensure exceptional care delivery
