Position Summary

The Hospice RN Case Manager – Homecare provides compassionate hospice care to patients in their homes and care facilities. This role is responsible for:

  • Conducting comprehensive nursing assessments
  • Developing and implementing individualized plans of care
  • Maintaining the highest level of patient comfort and quality of life
  • Coordinating care through an interdisciplinary team approach
  • Performing ongoing reassessments and revising care plans as patient/family needs evolve
  • Supervising and collaborating with team members to ensure exceptional care delivery
