*Job Description:* We are seeking a compassionate and dedicated *Licensed Vocational Nurse (LVN)* to provide one-on-one, in-home care for a 40 year-old patient in Inglewood, CA. boarder line Los Angeles, CA.
This role offers the opportunity to build a meaningful connection with your patient while delivering high-quality, individualized care in a supportive environment. *Patient Care Needs:*
- G-tube
- 126 lbs. need bed bath, diaper change
- Seizure monitoring and care
- non verbal
- Mobility assistance (patient uses a wheelchair)
- patient uses Hoyer lifter , a nurse will assist with lifting
- for bed bath another will assist LVN within a home

Responsibilities
*
- Provide skilled nursing care in accordance with the established plan of care
- Accurately chart and document all care using Axxess
- Monitor patient condition and report any changes promptly
- Maintain a safe, respectful, and comfortable environment for the patient
- Communicate effectively with the patient and family (Spanish required)

Requirements

- Active California LVN license)
- CPR/BLS certification, Physical, TB
- Experience with seizure care and suctioning is preferred, but not required
- Strong sense of responsibility, compassion, and professionalism
